Dimension value or Fact table

I am creating a data mart for a healthcare organization. Each patient is assigned a primary nurse. I need to be able to see total visits by primary nurse. So far I have patient and staff dimensions and a visit fact table. In a relational database I would have a patient-staff table that would show the relationship between the staff and the patient but I didn't think this was appropriate in a data warehouse. Any ideas on how to setup this up?

That would be your fact table. It holds all the keys and the event.
